16. Acute Icterohepatitis

Name: Chen.

Sex: Male.

Age: 22 years.

Occupation: A college student.

Chief Complaint: Abnormal liver-function for 2 days.

History of Present Illness: Two days ago, the patient took part in the physical examination for new college-students. After physical examination it was found that his liver function was abnormal: ALT was 53U/L, total bilirubin was 53umol/L, direct reacting bilirubin was 12.8umoL/L. The patient himself said sometimes there were abdominal fullness and bitterness of mouth, but there weren't obvious anorexia, nausea, vomiting and aversion to greasy food. Usually he had good sleeping and his urine was brown-coloured. Then his teacher ordered him to quit the college because of his abnormal liver-function. But he came to our hospital for treatment on September 12,1996.

Physical Examination: The patient was conscious with a sallow complexion. The pulse was taut and slippery, the tongue was fresh red with a thin layer of white fur. The sclera and the skin all over the body were ictericious. The lungs and heart were all normal. The abdomen was soft and flat, the spleen and the liver couldn't be palpated in the both inferocostal region. There was not other abnormal sign.

Examination of Liver Function: The abnormal results of liver function were as follow: ALT was 53U/L, total bilirubin was 53umo1/L,direct reacting biliruhin was 12.8umoUL. There was not other abnormal result.

Diagnosis: Acute Icterohepatitis.

Differentiation: This disease, in TCM, belongs to the category of "HuangDan(jaundice). " It was caused by dampness-heat evil, which attacked liver and gallbladder, and caused blood stasis and stagnation of qi. Then there was a failure of liver and gallbladder to conduct dipersive function normally, so that *here was retention of bile in the body, and the jaundice all over the body was caused.

Therapeutic Principle: Clearing away heat and removing toxin, activating blood and expelling dampness, counteracting of toxic materials and elimination of foulness; normalizing the secretion and discharge of bile to abate jaundice.

Recipe: Decoction for Protecting liver Formulated by myself.

ShengDiHuang,Dried Rehmannia Root 20g

MuDanPi,Tree Peony Bark 15g

LongDanCao,Gentian Root 18g

QiYeYiZhiHua,Windpipe Trachea 30g

DanShen,Red Sage Root 25g

JiangHuang,Turmeric Rhizome 15g

DaQingYe,Isatis Leaf 15g

BaiHuaSheSheCao, Oldenlandia 30g

YinChen,Oriental Wormwood 20g

HuZhang,Giant Knot-weed Rhizome 30g

BanLanGen,Isatis Root 15g

WuWeiZi,Schisandra Fruit 30g

WuMei,Black Plum 25g

ZhiZi,Capejasmine Fruit 20g

ShengDaHuang,Raw Rhubarb 12g (to be decocted later)

All the above drugs were to be decocted in water for oral taking. 10 doses were prescribed. Hot and greasy food was forhidden during the treatment.

Effect Observation: After 10 days' treatment, reexamination showed that: ALT was negative, total bilirubin was less than 3urnoL/L, there wasn't direct reacting bilirubin. The selera and the skin weren't icteritious and there was no uncomfortable sensation anymore. Then he was told to nurse himself with some heat-clearing drugs and liver-protecting drugs. After 2 years' follow-up, the sickness didn't recur. He had been in process of his college-living all along. Then it' s ending.
