14. Chronic Enteritis(2)

Case 2

Name: Qian.

Sex: Male.

Age: 50 years.

0ccupation: A worker.

Chief Complaint: Diarrhea for more than 20 years.

History of Present Illness : More than 20 years ago, the patient got diarrhea from an improper diet. Since then the sickness had been used to coming on again and again with soup like or watery stool. Weather changing and overexertion could induce it. When the illness came on, the diarrhea occurred more than 10 times a day. Recently the diarrhea occurred again because of overstrain, it occurred more than 10 times a day, accompanied with watery stool and poor appetite. So that he dared not to eat any food, for even having a little food would be followed by the sounds of "Gu-Gu" in the abdomen, and less than 10 minutes later, the diarrhea would occur with soup-like stool, in which the rice that had been eaten just now could be seen clearly piece by piece. In the past 20 years the patient had asked innumerable doctors for treatment, but there was no obvious effect on him. Then he came to our hospital for treatment on May 2,1996.

Physical Examination: The patient was weak and emaciated with a murky grey complexion. The pulse was thready and soggy, the tip of the tongue was fresh red, the root of the tongue was covered with a greasy layer of yellow fur. The chest was normal. The abdomen was soft and flat, there was not other abnormal sign.

Diagnosis: Chronic Enteritis.

Differentiation: It belongs to the category of "XieXie" (diarrhea) in TCM. It was caused by defeciency of spleen-Yang and kidney Yang, which caused the upward flaming of asthenic fire and the downward retention of cold phlegm in the lower-Jiao and accumulation of phlegm dampness in the middle Jiao.

Therapeutic Principle: Different treatment according to the theory of Three-Jiao should be used together in order to clear away the flaming-up asthenic fire; eliminate the damp phlegm retained in the middle warmer; induce the fire back to its origin in order to invigorate the life fire and clear away the cold phlegm from lower-Jiao.

Recipe:

DangShen,Pilose Asiabell Root 30g

BaiZhu,Bighead Atractylodes Rhizome 25g

HuangLian,Coptis Root 8g

HuangQin,Scutellaria Root 10g

PaoJiang,Baked Ginger 12g

RouGui,Cinnamon Bark 12g

YiYiRen,Coix Seed 30g

CangZhu,Atractylodes Rhizome 20g

ZhiHuangQi,Moxa-prepared Milkvetch Root 30g

DangGui,Chinese Angelica Root 15g

HouPu,Magnolia Bark 15g

TuSiZi,Dodder Seed 20g

DaZao,Chinese-date 12 pieces

All the above drugs were to be decocted in water for oral taking. 5 doses were prescribed. Greasy and fat food was forbidden during the treatment.

Effect Observation: After taking the 5 doses, the diarrhea occurred 3 or 4 times a day, the stool became like paste, having some food couldn' t cause the immediate diarrhea. Then he was to1d to use the original prescription continuously. After taking 20 doses of the original prescription, the patient was cured completely. His defecation occurred 1 or 2 times a day, the stool was normal already.

After 2 months' follow-up, the patient' s spirit became good and his complexion was ruddy and lustrous all along. Then it' s ending.
