13. Chronic Bacillary Dysentery

Name: Bian.

Sex: Male.

Age: 52 years.

0ccupation: A worker.

Chief Complaint: Diarrhea for more than 4 months.

History of Present Illness: More than 4 months ago, the patient got chill, dizziness and diarrhea from a cold. The diarrhea occurred more than 10 times a day. The stools were like yellow soup with some red and white mucus in it. Sometimes he had burning sensation around his anus during his defecating. After taking some drugs to treat common cold and "Spiramycin, Norfloxacin" by himself, the dizziness and chill disappeared, but the diarrhea occurred as same as before and it was followed by abdominal distension, anorexia and noisy sensation in the stomach. 4 months of constant diarrhea caused the patient got fatigue and lassitude with aching and limpness of knees. Then he came to our hospital for treatment on March 13,1996.

Physical Examination: The patient was conscious with a weak general condition. The abdomen was flat and soft, bowel sounds occurred frequently and hyperfunctionally. There was no tenderness over the abdomen. The pulse was thready and soggy. The tongue was dark red with a thin layer of yellow fur. There was not other abnormal sign.

Diagnosis: Chronic Bacillary Dysentery.

Differentiation: It belongs to the category of " XieXie" (diarrhea)in TCM. It's caused by attack of exogenous wind-heat which was retained in the GI tract due to spleen-asthenia. Then it caused the essential substance from food couldn't be absorbed normally and then some of them was transformed into damp phlegm evil. So that this disease was cold-syndrome associated with heat syndrom, deficiency in origin and excess in super ficiality.

Therapeutic Principle: Invigorating spleen and expelling dampness, warming middle-Jiao and reinforcing the function of kidney for anti diarrhea. It was most important to use warming therapy and heat clearing therapy together.

Recipe:

DangShen,Pilose Asiabell Root 30g

gaiZhu,Bighead Atractylodes Rhizome 25g

YiYiRen,Coix Seed 30g

CangZhu,Atractylodes Rhizome 25g

PaoJiang,Baked Ginger 15g

HuangLian, Coptis Root 15g

HuangQin,Scutellaria Root 15g

HouPu, Magnolia Bark 15g

ZhiKe,Fruit of Citron 15g

WuMei,Black Plum 20g

FuLing, Tuckahoe 20g

TuSiZi, Dodder Seed 25g

guGuZhi,Psoralae Fruit 25g

GanCao,Liquorice Root 15g

BaiTouWeng,Pulsatilla Root 50g

All the above drugs were to be decocted in water for oral taking. 5 doses were prescribed. Hot and greasy food was forbidden during the treatment.

Effect Observation: After taking the 5 doses, the diarrhea was relieved from more than l0 times to 4 times a day, the abdominal distension and the noisy sensation in stomach disappeared completely. The appetite got normal. Then 15g of ChiShiZhi(Red Nalloysite) were added into the used prescription, all these drugs were to be decocted in water for oral administration, he was told to take 5 doses of this changed prescription. After taking the 5 doses, he defecated less than 3 times a day.

After 6 months' follow up, the patient was well all along. then it's ending.
