8. Sinus Tachycardia

Name: Wu.

Sex: Male.

Age: 31 years.

Occupation: A factory director.

Chief Complaint: Palpitation, chest oppression and excessive sweating for 1 month.

History of Present Illness: A month ago, the patient got palpitation and chest oppression due to his busy working. At the beginning, it occurred paroxysmally, and wasn't accompanied by dizziness, dim eyesight, amaurisis and so on. Later on, it occurred continuously, accompanied with excessive sweating, which occurred more severely in every evening. Furthermore, it could be easily aggravated by slight working. Finally his palpitation and chest oppression were always aggravated by some common daily working. After being treated in the local hospital, the above syndrome remained unchanged. Then he came to our hospital for treatment on September 25,1998.

Physical Examination: The patient was conscious with a normal general condition. His chest, hack, forehead, palms and the undersides of feet were all covered with sweat. The pulse was thready and rapid. The tongue was red with a thin layer of white fur. The respiratory sounds were rapid and harsh. No tale could be heard in the lungs. HR was 116 beats/rain, the cardiac rhythm was regular, there were not systolic murmur and diastolic murmur. There was not other abnormal sign.

Examination of Electrocardiogram: It showed sinus tachycardia.

Lab Test: The results of thyroid hormone were all in normal limits.

Diagnosis: Arrhythmia : Sinus Tachycardia.

Differentiation: This disease, in TCM, belongs to the category of "XinJi ZhengZhong (palpitation). " It' s syndrome due to deficiency of both heart-qi and heart Yin. Deficiency of heart qi directly caused the initial qi in chest could not flow smoothly and normally, so the patient got palpitation and chest oppression; deficiency of heart-Yin directly caused his excessive sweating, for sweat is closely associated with heart according to the basic theory of TCM.

Therapeutic Principle: Invigorating qi and nourish Yin, activating blood and regulating the initial qi; tranqulizing the mind with heavy material.

Recipe:

DangShen,Pilose Asiabell Root 30g

MaiDong, Ophiopogon Root 12g

FuShen,Poria with Hostwood 18g

WuWeiZi,Schisandra Fruit 25g

DanShen,Red Sage Root 25g

TanXiang, Sandalwood 12g

ChuanXiong, Chuanxiong Rhizome 15g

6uaLouPi,Trichosanthes Fruit 25g

ZhiHuangQi,Moxa-prepared Milkvetch Root 30g

DangGui,Chinese Angelica Root leg

XuanShen,Scrophularia Root 10g

ShengLongGu,Dragon's Bone 30g (to be decocted earlier)

YiYiRen,Coix Seed 30g

ShengDiHuang,Dried Rehmannia Root log

XieBai,Macrostem Onion 15g

ZhiGanCao,Prepared Liquorice Root 18g

All the above drugs were to be decocted in water for oral taking, 5 doses were prescibed. Hot and greasy food was forbidden during the treatment.

Effect Observation: After taking the decoction, there was no discomfort any more, he could run upstairs to the 4th floor easily and palpitation and chest oppression could not be induced, furthermore, the instant HR was 88 beats/min. There wasn't excessive sweat either. Then he was told to pay attention to nursing himself well and keep a proper balance between work and rest. After 1 year's follow-up, the patient was in good health all along, then it' s ending.
