Case 1

Name: Chert.

Sex: Male.

Age: 23 years.

0ccupation: A college-student.

Chief Complaint: Defecating-pain and hematochezia for 20 days.

History of Present Illness: Twenty days ago, the patient got defecating-pain and hematochezia without any obvious causes. The hematochezia was fresh red in colour and less in amount, accompanied with prickling-pain. After his defecating, the toilet paper was blood-stained. The stool was hard-textured and mass shaped. There was not other complaint. He came here for treatment on January 10,1997.

Physical Examination: The patient was conscious with a normal general condition. The pulse was thready and uneven, the tongue was red with a thin layer of white fur. At 12 o' clock of the anus, there was a 0. 5era long longitudinal laceration. There was not other abnormal sign.

Diagnosis: Anal Fissure.

Differentiation: It belongs to the field of "ChangFengXiaXue" (hematochezia) in TCM. It's caused by the damp-heat, which accumulated in the intestine and the anus. When the damp-heat fumigated and evaporated the intestine and the anus, the intestine and the anus became relatively dry and rough, then the friction between the stool and the anus hurt the anal mucosae and caused the anal fissure.

Therapeutic Principle: Moistening purgation, softening hardnessand to promote defecation, cooling blood and astringing the laceration.

Recipe:

CeBaiYe,Biota Tops 15g

HuaiJiao,Sophora Fruit 12g

WuMei,Black Plum 15g

ZhiKe,Bitter Orange 12g

HuoMaRen, Hemp Seed 10g

DangGui,Chinese Angelica Root 10g

XingRen,Apricot Kernel 10g

ShuDaHuang,Prepared Rhubarb 9g

DiYuTan,Calcined Sanguisorba Root 15g

All the above drugs were to be decocted in water for oral taking. 3 doses were prescribed. He was told to avoid drinking and eating hot and greasy food.

Effect Observation: After 3 days of the above treatment, the pain and hematochezia disappeared. After 4 months' follow-up, the sickness didn' t come back. Then it' s ending.

Case 2

Name: Zhou.

Sex: Female.

Age: 32 years.

0ccupation: A worker.

Chief Complaint: Hematochezia, defecating pain and constipation for 2 months.

History of Present Illness: The patient had gotten "Anal Fissure" for several years. She always got hematochezia, defecating-pain and constipation because of frequent attack of the sickness, so that she had to often go to the local specialized department of anal fistula for treatment. After injecting a drug whose name was unknown into the affected part, she could get better for a few days. 2 months ago the sickness recurred again, she got hematochezia defecating-pain and constipation again, then she came here for treatment on December 2,1997.
