18. Hydrocele of Tunica Vaginalis

Name: Yin.

Sex: Male.

Age: 25 years.

Occupation: A supply marketing person.

Chief Complaint: Distending pain of both testis for 1 month.

History of Present Illness: One month ago, the patient got a distending pain in the both testis without any obvious causes. Especially after having sexual intercourse, it occurred obviously severelier. Then he came to our hospital for treatment on July 21,1998. After the illness came on, he kept his appetite, sleeping, defecation and urination normal all along. There was not other complaint.

Physical Examination: The patient was conscious with a normal general condition. The pulse was thready and soggy. The tongue was pale, swollen and teeth printed with watery fur. The chest and abdomen were all normal. The appearance of both testis was normal. Two soft cystomas were touched in the both sides of the epididymide with slight tenderness. There was no abnormal sign else.

Examination of Multicoloured Ultrasonogram: Opaque dark area of fluid whose depth was 1.2cm was seen in the right tunica vaginalis of testis; opaque dark area of fluid whose deepth was 1.7cm was seen in the left tunica vaginalis of testis.

Diagnosis: Hydrocele of Tunica Vaginalis.

Differentiation: It's called " ShuiShan" ( watery hernia) in TCM. It was deficiency of spleen-qi, which caused physiologic fluid couldn't be transported normally in the body, then the fluid was accumulated in the scrutum and turned into phlegm-dampness.

Therapeutic Principle: Invigorating qi and benefiting spleen; eliminating pathogenic phlegm and water.

Recipe:

DangShen,Pilose asiabell Root 30g

BaiZhu,Big head Atractylodes Rhizome 18g

FuLing, Tuckahoe 15g

YiYiRen,Coix Seed 30g

ZeXie,Oriental Water Plantain 18g

JuHe,Orange Kernel tSg

XiaoHuiXiang,Common Fennel Fruit 12g

LiZhiHe, Lychee Seed 15g

RouGui,Cinnamon Bark 5g

NiuXi,Achyranthes Root 12g

ZhuLing,Umbellate Pore Lungus 15g

MaoZhuaCao,Ternate Buttercup Root 20g

XuDuan, Teasel Root 18g

TuFuLing,Smilax Glabra Rhizome 20g

ZhiGanCao,Prepared Liquorice Root 10g

All the above drugs were to be decocted in water for oral taking. 5 doses were prescribed. Cool and cold food was forbidden during the treatment.

Effect Observation: After taking the above decoction, he came back for reexamination. The reexamination of multicoloured ultrasonogram showed the depth of both sides of hydocele was 0.3cm. It showed the illness was mostly cureded. Because his tongue was pale with watery fur yet, then it's necessary to add 8g of ZhiFuZi (Prepared Aconite Root) and 20g of BaJiTian (M0rinda Root) omit 20g of TuFuIAng(Smilax Glabra Rhizome) and 15g of ZhuLing (Umbellate pore Lungus) in the original recipe in order to invigorate Yangqi. All these drugs were to be decocted in water for oral taking. 3 doses of this changed prescription were prescribed. After taking the decoction, the patient's tongue and pulse became normal. Then it's ending.
