Chief Complaint: Sudden swelling pain of right buttock for 1 day after being injected.

History of Present lllness: Half a month ago, the patient got a common cold, then she went to the local clinic and asked the barefoot doctor for treatment. She was treated by intramuscular injecting "Benzylpenicillin" and oral taking some common antibiotics. After 6 days' treatment, she got well. But 1 day ago, she had a sudden swelling-pain in her right buttock without any obvious causes. It was a severe swelling-pain and progressively got worse and worse so that she could not sit, stand, walk and sleep normally. Then she came to me for treatment on February 10,1997. Since the illness came on, her urination and appetite had been all normal, while her sleeping was disturbed by the severe swelling-pain throughout night. In addition, because it was in the Spring Festival at that time, the patient had taken too much hot and greasy food during the period. Her stool was dry and costive. There was not other complaint.

Physical Examination: The patient was conscious with an Mute painful complexion. The pulse was rapid and floating, the tongue was deep red with a thick layer of white fur. The chest and abdomen were all normal. Partial examination showed the 1^st quarter of the right buttock got severe swelling, its colour was purplish red, the size of the affected region was 8cm¡Á 8cm. There was so sharp tenderness in the affected region that it couldn' t be palpated. The skin of the affected region was obviously hotter than normal. There was not other abnormal sign.

Diagnosis: Deep Infection After Intramuscular Injection. came to me in a flurry, he told me that the swelling pain got more severe than before. Physical examination showed the affected region enlarged from 8cm¡Á8cm to 10cm¡Á10cm, while its colour became from purplish red to fresh red. This sign showed the concealed evil had been defeated and dispersed and the deep abscess had been wrecked, it indicated the decoction was suitable for the illness. In order to relieve the swelling-pain quickly, then I to1d them to apply some external drugs onto the affected region.

Recipe:

ShengDaHuang, Raw Rhubarb

SheXiang, Musk

HuangLian, Coptis Root

ChanSu,Toad Venom

BingPian, Borneol

XiongHuang, Realgar

(Each herb was taken just the proper amount. )

All the above drugs were to be ground into fine powder, they should mix the powder with some honey to make herb plaster, applying some herb plaster onto the affected region, once a day. In the same period, they was told to go on taking the original decoction.

After 3 days' treatment, the swelling-pain was relieved greatly. Then it's necessary to decrease ShengDaHuang (Raw Rhubarb)from 12g to 9g in the original prescription, the other drugs were as same as before. They was told to take 5 doses of this changed prescription for oral taking and stop applying the herb plaster, avoiding eating hot and greasy food during the treatment.

After taking the 5 doses, the patient got well completely. After 3 months' follow up, the illness didn't recur. Then it's ending.
