9. Rhinorrhagia

Case 1

Name: Guo.

Sex : Female.

Age: 26 years.

Occupation: A teacher.

Chief Complaint: Intermittent nosebleed for more than 20 days.

History of Present Illness: More than 20 days ago, the patient got an intermittent nosebleed without obvious causes. It's less in amount and could be relieved by putting a cold towel onto the forehead or having a rest. It mostly occurred in morning and evening 2 or 3 times a day. After half a month's treatment of taking "VitC, Adrenosem" and "Etamsylati" in the local policlinic, there was no effect on her. Then she came here for treatment on October 25,1997. The patient was in good health as usual. She kept her appetite, defecation, urination and sleeping normal all along. Only occasional dizziness and vexation were there after the sickness came on. There wasn't other complaint.

Physical Examination: The patient was conscious with a normal general condition. The complexion was fresh red. The pulse was thready and floating. The tongue was fresh red with a dry layer of white fur. The nasal mucosa was congestive, there weren't erosion and pura. There wasn't other abnormal sign.

Diagnosis : Rhinorrhagia.

Differentiation: It's deficiency of lung-Yin and kidney Yin, which caused the up-flaming of deficiency-fire and burnt the tiny blood vessels and collaterals in the nose. Then there appeared an intemittent nosebleed.

Therapeutic Principle: Clearing away heat and nourishing Yin.

Recipe: Bolus of Anemarrhenae, Phellodendi and Rehmanniae.

ZhiMu,Windweed Rhizome 15g

HuangBai, Phellodendron Bark 15g

MuDanPi,Tree Peony Bark 20g

ShuDiHuang,Prepared Rehmannia Root 18g

ShanYao,Chinese Yam Rhizome 15g

FuLing, Tuckahoe 10g

ZeXie,Oriental Water Plantain Rhizome 12g

ShanYuRou,Dogwood Fruit 18g

2 doses were prescribed, all the drugs were to be ground into fine powder, and mixed with honey together to make pills, she should take 9g of the pills each time, 3 times a day. Greasy and hot food was forbidden during the treatment.

Effect Observation: After 5 days of the treatment, the nosebleed disappeared, while there were occasional dizziness and vexation yet. It showed the asthenic fire had been sent down while there was the deficiency of Yin yet. Then she was suggested to continue the treatment. After 1 and a half years' follow up ,the dizziness, vexation and nosebleed disappeared and the illness didn't recur. Then it's ending.

Case 2

Name: Cai.

Sex: Male.

Age: 23 years.

Occupation: A computer designer.

Chief Complaint: Intermittent nosebleed for more than 1 month.

History of Present Illness: More than 1 month ago, the patient got an intermittent nosebleed without any obvious causes. It occurred in both sides of the nose cavity and was sometimes more and sometimes less in amount. It could be relieved by resting, laying down or putting a cold-towed onto the forehead. Its frequency was not fixed in a day, mostly occurring in morning and evening, occasionally occurring in other time. Then he went to hospital for treatment, it was diagnosed as "Rhinorrhagia'" and he was treated with "VitC, VitBco" and some common haemostatic drugs. After 20 days' treatment, not only the syndrome wasn't relieved, but also its occurring became more and more frequent. It's often induced by overworking or eating. Then he came here for treatment on August 4, 1999 through somebody. After the illness came on, she kept her urination. defecation, appetite and sleeping normal all along. There was not other complaint.

Physical Examination: The patient was conscious with a normal general condition. The tongue was red and thin with a thin layer of white fur. The pulse was taut and rapid. The nasal mucosa was congestive, there weren't erosion, pura and other abnormal sign. There was not other abnormal sign.

Diagnosis : Rhinorrhagia.

Differentiation: It's deficiency of liver-Yin and kidney-Yin, which couldn't support liver-Yang and kidney-Yang as normal. So that the asthenic fire flamed upward and burnt the tiny blood vessels and collaterals of the nasal cavity. Then there was an intermittent nosebleed.

Therapeutic Principle: Clearing away heat and nourishing Yin.

Recipe: Blous of Anemarrhenae,Phellodendi and Rehmanniae.

Two doses were prescribed, all the drugs were to be ground into fine powder and mixed with some honey together to make pills. He was told to take 9g of the pills each time,3 times a day. Greasy and hot food was forbidden.

Effect Observation: After 5 days of the treatment, the nosebleed disappeared. It took 24 days totally to be in process of the treatment. After half a year' s follow up, the illness didn' t recur. Then he was told to pay attention to having good rest and nursing himself well. Then it's ending.
