8. Acute Nasosinusitis

Name: Long.

Sex : Female.

Age: 27 years.

Occupation: An attendant.

Chief Complaint: Swelling-pain of dextral facies for 3 days.

History of Present Illness: Three days ago, the patient scratched her nasal cavity with her finger because of an itching sensation in it. A few hours later, her dextral facies became severely swelling, accompanied with distending pain. Then she went to the department of otorhinolaryngology in hospital for treatment. After physical examination, it was diagnosed as "Acute Rhinitis," and she was treated by intravenous dripping antibiotic drugs. After 2 days' treatment, the distending pain was relieved while the dextral facies was as swelling as before. Moreover, it's difficult for her to swallow food or say a word, and she got a sensation of being obstructed in the nasal cavity. Then she came here for treatment on October 19,1998.

Physical Examination: The temperature was 37.2. The patient was conscious with a normal general condition. The pulse was thready and floating; the tongue was red with a thin layer of white fur. The dextral facies and the dextral wing of nose were severely swelling. If touched upon, there was tenderness. The dextral mycteric mucosa was diffuse swelling and scarlet with congestion. There wasn't other abnormal sign.

Diagnosis: Acute Nasosinusitis.

Differentiation: It's caused by the dirt in the nail groove, which damaged the collaterals of the nose. Then the pathogenic dampness-heat got into the subcutaneous layer directly. So that blood stasis and stagnation of qi was formed, which caused the swelling and distending pain of the dextral facies.

Therapeutic Principle: Clearing away heat to remove toxin, activating blood to resolve stasis; removing the stasis in the collaterals to induce subsidence of swelling.

Recipe:

XuanShen,Scrophlaria Root 25g

DanShen,Red Sage Root 18g

ShanDouGen,Subprostrate Sophoia Root 12g

BaiJiangCan,Batryticated Silkworm 10g

DiLong, Earthworm 15g

QiangHuo,Notopterygium Root 12g

BanBianLian,Radical Lobelia 15g

BanZhiLian,Barbat Skullcap 15g

BoHe,Peppermint Herb 12g (to be decocted later)

ChanYi,Cicada Slough 12g

RenDongTeng, Honeysuckle Stem 15g

YeJuHua,Mother Chrysanthemum 15g

JieGeng,Platycodon Root 18g

ShengGanCao,Raw Liquorice Root 6g

All the above drugs were to be decocted in water for oral taking. 3 doses were prescribed. Greasy and hot food was forbidden during the treatment.

Effect Observation: After taking the 1st dose, the subsidence of swelling was induced greatly, after taking the 2nd dose, the dextral facies became as normal as before. Then she was told to take the 3rd dose to eliminate the remnant of the evils in the collaterals. After 1 week's follow-up, the illness didn't recur. Then it's ending.
