Age: 48 years.

Occupation: A peasant.

Chief Complaint: Sudden deafness and tinnitus for 9 years.

History of Present Illness: Nine years ago, the patient got a sudden fall of her audition without any obvious causes, accompanied with tinnitus of ears. The tinnitus was a low tone of rumble, it occured all day and night without intermission. It prevented her from hearing normal voice from others and talking with others. When someone wanted to say a word to her, only by putting the mouth near her ear and shouting loudly to her, could the voice be heard by her with great difficulty. In the past 9 years, she had gotten innumerable treatment, she had separately been to some hospitals in Jiangsu Province for treatment, but there was no curative effect on her. After many kinds of lab tests in the hospitals, it showed that the bone conduction and air conduction were lower and the tympanic membrane of the sinister ear subsided, there wasn't other abnormal sign. Then the illness was diagnosed as "Nervous Deafness and Tinnitus. "After the illness came on, the patient couldn't have normal social intercourse for 9 years, so that she became obtuser and obtuser while her sleeping, defecation and urination were all normal. She came to our hospital for treatment on December 1,1995 through somebody.

Physical Examination: The patient was obtuse with an apathic complexion. The pulse was deep and uneven. The tongue was pale with a thick layer of white fur. The chest and abdomen were all normal. Both aures were normal, otopiesis was seen in the sinister ear. There wasn't other abnormal sign.

Diagnosis: Nervous Deafness and Tinnitus.

Differentiation: It's deficiency of liver and kidney, so that qi and blood were too deficient to flow upward into the ears and then ears couldn't be nourished normally. In addition, the ears were obstructed by exogenous pathogenic wind-dampness. So that the patient lost her audition and got a tinnitus.

Therapeutic Principle: Nourishing liver and kidney, invigorating primordial qi, reinforcing qi and blood in order to promote them flowing upward into ears, keeping the ears clear and unobstructed by aromatic herb, activating blood to resolve stasis; expelling wind and expelling dampness.

ZhiHuangQi,Moxa prepared Milkvetch Root 30g

DangGui,Chinese Angelica Root 15g

GouQiZi,Wolfberry Fruit 25g

BaiShao,White Peony Root 20g

YuanZhi,Polygala Root 25g

ShiChangPu,Grassleaved Sweetflag Rhizome 20g

DiLong, Earthworm 15g

BaiJiangCan,Batryticated Silkworm 15g

GaoBen, Ligusticum Root 15g

All the above drugs were to be decocted in water for oral taking. 10 doses were prescribed. I told her to take a little white spirit as a guiding herb to enhance the efficacy of the decoction. In addition,I told her to take lg of the powder of SuHeXiang, Storax each time, once a day. In the same period, lml of injection of DangGui, Chinese Angelica Root was injected into the points of SJ17 ,Yifeng (both sides),0.5ml each point, once a day.

Effect Observation: After 10 days of the above treatment, the patient's audition was improved and the tinnitus was relieved. She became sensitive with a bright complexion. If someone stood at a place lm from her and spoke to her in normal volume, some words in the sentence could be heard by her, if the voice got louder a little, all the sentences could be heard clearly by her. Then the point-injection was used as before, I added 20g of MuXiang(Aucklandia Root), 15g of XiXin ( Asarum Herb) and 15g of ZaoCi(Chinese Honeylocust), omit 20g of BaiShao(White Peony Root), 10g of QiangHuo(Notopterygium Root) and 15g of GaoBen(Ligusticum Root) in the original prescription. 5 doses of this changed prescription were prescribed, all the drugs were to be decocted in water for oral taking, I told her to take a little white spirit as a medical supplement to enhance the efficacy of the decoction and stop using SuHeXiang(Storax).

After 5 days of the above treatment, the patient's audition was improved obviously, but there were an uncomfortable sensation in the stomach and an obstructed sensation in the throat during the treatment, then it's necessary to use the 1^st prescription again instead of the 2nd prescription, for the 2nd prescription unfitted the patient's stomach for working normally. 10 doses of the 1st prescription were precribed, all the drugs were to be decocted in water for oral taking. The point injection was used continously.
