6. Nervous Tinnitus

Name: Sun.

Sex :Fernale.

Age: 23 years.

Occupation: A college student.

Chief Complaint: Tinnitus and distending pain of the dextral ear for 1 month.

History of Present Illness: One month ago, the patient had tinnitus in her dextral ear caused by overexertion and catching a cold, accompanied with distending pain in the ear. The tinnitus was a low tone of rumble. At the beginning, it occurred intermittently, later on, it occurred constantly all night and day, accompanied with distending pain, dizziness, asthenia, rhinorrhea and dryness of throat. Then she went to the local policlinic for treatment. The illness was diagnosed as "Common Cold, "and she was treated with the "powder of ZhengCaiHu, Spiramycin" and so on. After 1 month of the above treatment, rhinorrhea and dry throat was relieved, while the dextral ear was ringing continuously, accompanied with constant distending pain. Then

she came here for treatment on October 20,1997.

Physical Examination: The patient was conscious with a normal general condition. No abnormal sign was found in the throat and pharynx. The pulse was thready and deep. The tongue was pale with a thick layer of white fur. The dextral auricle was normal, the dextral antrum auris was unobstructed. The tympanal membrane was intact, but its umbilical region subsided inside. There wasn't other abnormal sign.

Diagnosis: Nervous Tinnitus.

Differentiation: It's caused by attack of exogenous cold dampness, which got into the ear due to deficiency of spleen-Yang. Western medicine considered that the tinnitus and the distending-pain were caused by obliteration of auditory tube.

Therapeutic Principle: Relieving the primary and secondary symptoms together, expelling cold-dampness in order to relieve the secondary symptom, invigorating spleen Yang in order to cure the primary symptom.

Recipe: Modified Decoction for Strengthening middle Jiao and Benefiting Qi and Decoction of Agastachis for Restoring Healthy qi.

ShengHuangQi,Raw Milkvetch Root 30g

DangGui,Chinese Angelica Root 12g

DangShen,Pilose Asiabell Root 30g

BaiZhu,Bighead Atractylodes Rhizome 18g

ShengMa,Cimicifuga Rhizome 8g

ChaiHu,Bupleurum Root 3g

ChenPi,Tangerine Peel 15g

HuoXiang,Agastache Aerial Parts 18g

BaiZhi,Dahurian Angelica Root 12g

HouPu, Magnolia Bark 9g

FuLing, Tuckahoe 12g

JieGeng,Platycodon Root 15g

SuYe,Perilla Leaf 12g

FaBanXia,Pinellia Tuber 8g

ZhiGanCao,Prepared Liquorice Root 10g

2 doses were prescribed. All the drugs were ground into powder and mixed with water to make pills, she was told to take 6g of the pills each time,3 times a day. In addition, because the distending-pain in the ear was too severe currently, then it's necessary to relieve it at once by acupuncture.

Acupuncture Method: The point of Yemen(TE2) was punctured with a 1.5 cun needle, I manipulated the needle with reducing skill.

Effect Observation: After one time's acupuncture, the distending-pain disappeared at once. After 15 days of the above treatment, the tinnitus disappeared, the patient became spirited. After 2 months of follow-up, the illness didn't recur. Then it's ending.
