4. Pharyngolaryngeal Neurosis

Name: Huang.

Sex : Male.

Age: 20 years.

Occupation: A college-student.

Chief Complaint: Uncomfortable sensation of throat for 5 years.

History of Present Illness: Five years ago, the patient got an ncomfortable sensation of throat without obvious causes. It eemed that his throat was obstructed by something, which could ot be swallowed down and expectorated out. Especially in every morning and every evening, the uncomfortable sensation got more obvious. Meanwhile it wasn't accompanied by constant cough and diffuse sputum and so on. In the past years, he had been to several hospitals for treatment. The illness was diagnosed as "Chronic Bronchitis, "he was treated with anti-inflammatory, phlegm-resolving and antitussive drugs, but there was no effect on him. Then he came to our hospital for treatment on November 18,1997.

Physical Examination: The patient was conscious with a normal general condition. The pulse was thready and soggy. The tongue was dark purple with a thin layer of white fur. No abnormal sign was found in the throat. There wasn't other abnormal sign.

Diagnosis: Pharyngolaryngeal Neurosis.

Differentiation: This disease, is called "Mei He Qi" (globus hystericus) in TCM. It's caused by pathogenic fire and phlegm, which got entangled each other due to spleen-asthenia and liver-depression, and then they attacked the throat, so that the patient got the uncomfortable sensation in his throat.

Therapeutic Principle: Invigorating spleen and dispersing liver, eliminating pathogenic fire-phlegm.

Recipe :

Dangshen,Pilose Asiabell Root 30g

BaiZhu,Bighead Atraetylodes Rhizome 15g

FuLing, Tuckahoe 15g

ShengShanZha,Raw Hawthorn Fruit 20g

HuangLian,Coptis Root 9g

GuiZhi,Cinnamon Twig 12g

ChenPi,Tangerine Peel 18g

ShengLongGu,Dragon's Bone 25g (to be decocted earlier)

ChaiHu,Bupleurum Root 5g

ShengMa,Cimicifuga Rhizome 6g

ZhiShi,Immature Bitter Orange 15g

FaBanXia,Pinellia Tuber 12g

JieGeng,Platyeodon Root 15g

ShengGanCao,Raw Liquorice Root 10g

All the above drugs were to be decoct in water for oral taking. 3 doses were prescribed. After taking the decoction, the uncomfortable sensation in the throat was relieved greatly. Because it's not convenient for the patient to take the decoction, then I told him to take 2 doses of the above prescription and grind the drugs into powder, ix the powder and water together to make pills, e should take 6g of the pills each time, 8 times a day.

Effect Observation: After 20 days of the above treatment, the uncomfortable sensation of throat disappeared. After 3 months' follow-up, the sickness didn't recur. Then it's ending.
