Name: Yang.

Sex: Female.

Age: 2*(3/12) years.

Chief Complaint: Cough and asthma for 1 month.

History of Present Illness: One month ago, the patient got cough and rhinorrhea caused by catching a cold. Then she was led to the local policlinic for treatment, she was treated with the "powder for quick treating children's common cold, the powder for relieving children's cough," and "Ampicillinum Natricum" etc. After several days' treatment, her rhinorrhea disappeared while her cough wasn't relieved. Furthermore, there were whitish sticky sputum and constant asthma. Sometimes her serious asthma caused her mouth breathing, but there weren't cyanosis, hemoptysis, high fever and night sweat etc. Then she was sent to hospital and was treated by intravenous dripping "Penicillin G" for 1 week, but there was no effect on her. Then she was sent to a hospital in Nanjing and was treated by intravenous dripping "Penicillin G, Aminophylline "and oral taking the "oral liquid of loquat leaf and Sichuan Fritillary Bulb, ect. for relieving cough "for more than 20 days, but there was no effect on her either. The baby coughed and gasped without intermission all day and night. Then her parent led her to me for treatment on December 29,1995.

Physical Examination: The patient was conscious with mouth breathing. There were obvious wheezing sounds which could be heard clearly in her rapid breathing without using the stethoscope. The respiratory sounds were harsh and rapid, there were wheezing sounds and dry tales all over the lungs. The thready and rapid pulse was found through single finger pulse feeling. The tongue was red with a thick layer of white fur. There wasn't other abnormal sign.

Examination of Chest Roentgenogram: It showed the bronchovascular shadows were increased and disorderly. There weren't other abnormal findings.

Diagnosis: Asthmatic Bronchitis.

Differentiation: This disease, in TCM, belongs to the category of "KeSou"(cough) and "XiaoChuan"(asthma). It was caused by wind-cold attacking the exterior and retention of phlegm-dampness from the interior. The pathogenic wind and cold prevented pulmonary qi from taking charge of dispersing function and then the constant cough was caused; the phlegm-dampness obstructed the lungs and then the frequent asthma was caused. Additionally, the phlegm-dampness obstructed the middle Jiao and restrained spleen-qi, then the restrained spteen-qi couldn't normally conduct the function of transmission. So that it was more difficult to eliminate the phlegm-dampness in the lungs and the cough and asthma couldn' t be cured easily.

Therapeutic Principle: Invigorating spleen to eliminate phlegm-dampness; regulating pulmonary-qi to smooth chest and stop the asthma.

BaiZhu,Bighead Atractylodes Rhizome 10g

CangZhu,Atractylodes Rhizome 6g

FuLing, Tuckahoe 8g

ChenPi,Tangerine Peel 5g

FaBanXia,Pinellia Tuber 4g

ZhiKe,Fruit of Citron 4g

SuZi,Perilla Fruit 5g

BaiJieZi,White Mustard Seed 5g

LaiFuZi,Radish Seed 5g

GanCao,Liquorice Root 3g

All the above drugs were to he decocted in water for oral taking. 3 doses were prescribed.

Effect Observation : After taking the decoction, the asthma and the cough were relieved greatly, the patient got better than before obviously, she could breathe smoothly without mouth breath, her respiratory sounds became smooth and clear, the wheezing sounds disappeared. Some dry rales could be heard at the end of inspiration. All these signs showed the used decoction was obviously effective. But it took many days to be in the illness, the permanent cough and asthma always hurt the pulmonary qi and kidney qi. Then it's necessary to reinforce eliminating pathogenic dampness phlegm and restraining excessive consumption of kidney-qi and lung qi.

All the above drugs were to be decocted in water for oral taking. 3 doses were prescribed.

After taking the decoction, the asthma and the cough disappeared, physical examination showed the respiratory sounds were smooth and clear, no rale and wheezing sound could be heard in lungs. Then her parents were told to nurse her well and protect her from catching a cold. After 2 years' follow-up, the illness didn' t recur. Then it's ending.
